To keep customers happy you need to deliver time after time. If a problem arises notify your customer as soon as possible. Explain how you are going to rectify the situation and then follow through to ensure positive results. This also includes being timely with invoices and correspondence. You might consider creating a system to ensure that each task is performed in a correct manner consistently.
